Day Break Adult Day Health Care

Day Break Family Services

We are a real estate holding company with that manages multiple single family, multi-family, commercial and other special purpose properties covering the length of the Golden State, stretching from Northern to Southern California. We also offer short to long term rental from Airbnb/Vrbo to long-term tenants.

At the core of our work is our commitment to delivering high-quality Adult Day Health Care services to underserved communities in Merced County. Our center is a lifeline for individuals who face complex medical, cognitive, or physical challenges—many of whom would otherwise fall through the cracks of the healthcare system. We provide a structured, supportive environment where participants receive skilled nursing care, physical and occupational therapy, mental health services, social work support, and nutritious meals—all in one welcoming space. More than a program, this is a place of healing, connection, and dignity—designed to empower our participants to maintain independence, reduce hospitalizations, and live fuller lives. We serve not only individuals, but families and caregivers, offering peace of mind through trusted, compassionate care.

Our Enhanced Care Management (ECM) program is a cornerstone of our commitment to supporting the most vulnerable Medi-Cal members across Merced, Madera, Mariposa, and San Benito Counties. Through this CalAIM initiative, we provide whole-person, community-based care coordination for individuals facing complex health and social challenges. Our dedicated Care Managers work one-on-one with members to navigate fragmented systems, connect them to vital services—such as housing support, mental health care, medical appointments, and social services—and advocate for their well-being every step of the way. ECM is not just about access to care; it’s about building trust, restoring dignity, and ensuring that no one is left behind. We meet our members where they are—physically, emotionally, and culturally—because we believe that everyone deserves to be seen, heard, and supported on their path to better health.
